Basic Life Support
Annual Report must be submitted NLT 30 September of current fiscal year.
Semi-Annual Report is due NLT 31 Mar
Advanced Cardiac Life Support / Pediatric Advanced Life Support
Annual Report (Current Fiscal Year) and Pre-Course Approval Requests must be submitted to the MTN NLT 30 September for
the upcoming fiscal year.
Post-Course Reports are due to the MTN NLT 30 days after the course was
conducted.
IMPORTANT NOTICE
Please note that if the proper documentation is not submitted to the Military Training Network in a timely manner, your program's authority to conduct training under the Military Training Network, will be in jeopardy.
