All Activity Directors and/or staff who worked directly with the USU CE activity manager is requested to submit this customer service evaluation form. The information provided is not shared directly with the specified staff person, but is used to improve our customer service and assure we are maintaining a high caliber of service to our users. If problems are not reported, they will continue to occur in the future, so your candid responses are valued. If multiple Activity Director staff worked with the CE office, multiple evaluation forms can be submitted.
